ABSTRACT

Abstract Objective: The aim of this study was to evaluate a flapless surgical technique as an alternative to traditional alveolar corticotomy used to accelerate orthodontic tooth movement (OTM). Methods: To induce OTM in Wistar rats, 40 cN of orthodontic force were applied to the maxillary left first molars. Forty rats were distributed into control groups (CG1, CG3, CG7 and CG14) and experimental groups (n= 5), in which alveolar perforations were made using a spear-shaped guide bur (EG1, EG3, EG7, EG14). Euthanasia dates were set at 1, 3, 7 and 14 days, respectively, after tooth movement began. The amount of OTM was measured with a caliper, and osteoclasts present in the periodontal ligament of the mesial root of the moved tooth were counted by means of histological evaluation (tartrate-resistant acid phosphatase staining, TRAP). Results: Although there was no difference in the amount of OTM within subgroups of corresponding experimental periods (p> 0.05), when EG14 and CG14 were compared, a larger number of osteoclasts was counted in the experimental group (p< 0.00). Conclusion: The authors concluded that flapless cortical alveolar perforations led to more intense osteoclastic activity on the fourteenth day; nevertheless, no evidence of accelerated OTM could be noted.


Resumo Objetivo: o objetivo do presente estudo foi avaliar a técnica cirúrgica sem retalho como alternativa à tradicional corticotomia alveolar utilizada para acelerar o movimento dentário experimental. Métodos: para induzir a movimentação dentária experimental em ratos Wistar, foram aplicados 40 cN de força ortodôntica aos primeiros molares superiores esquerdos. Quarenta ratos foram distribuídos nos grupos (n = 5) controles (GC1, GC3, GC7 e GC14) e experimentais (GE1, GE3, GE7, GE14), nos quais foram realizadas perfurações alveolares com uma ponta lança. As datas de eutanásia foram estabelecidas em 1, 3, 7 e 14 dias, respectivamente, após o início do movimento dentário. A quantidade de deslocamento dentário foi medida com um paquímetro e os osteoclastos presentes no ligamento periodontal da raiz mesial do dente movimentado foram contados por meio de avaliação histológica (coloração por fosfatase ácida resistente ao tartarato, TRAP). Resultados: embora não tenha havido diferença na quantidade de deslocamento dentário dentro dos subgrupos dos períodos experimentais correspondentes (p> 0,05), quando GC14 e GE14 foram comparados, um número maior de osteoclastos foi contado no grupo experimental (p< 0,00). Conclusão: os autores concluíram que as perfurações alveolares corticais sem retalho levaram a uma atividade osteoclástica mais intensa no décimo quarto dia; entretanto, nenhuma evidência de movimento dentário acelerado pôde ser notada.

ABSTRACT

Abstract The aim of this study is to evaluate the action of paramonochlorophenol associated with Furacin followed by calcium hydroxide (CH) dressing in the control of inflammatory root resorption in cases of immediate tooth replantation with delayed endodontic treatment. A total of 28 incisors of 3 male dogs were extracted and replanted after 15 minutes, and randomly divided into 3 groups: Group I (n = 8) - endodontic treatment was performed before the extraction and replantation; Group II (n = 10) - endodontic treatment was performed 30 days after replantation and the root canal was filled with CH dressing; Group III (n = 10) - endodontic treatment was performed 30 days after replantation and root canals received temporary medication of paramonochlorophenol-Furacin followed by CH dressing. The animals were euthanized 90 days after replantation. The histomorphological events analyzed at the epithelial reattachment site were the intensity and extent of acute and chronic inflammatory processes, periodontal ligament (PDL) organization, the intensity and extent of acute and chronic inflammatory processes in the PDL space, root resorption, bone tissue, and ankylosis. Data were submitted to the Wilcoxon Signed Ranks Test for group comparison (α = 5%). In Groups I, II and III the periodontal ligament was regenerated and most of the resorption areas were repaired by newly formed cementum. The depth and extent of root resorption were significantly higher in Group II than in Group III. The use of paramonochlorophenol-furacin followed by CH dressing was more effective in controlling inflammatory root resorption after immediate tooth replantation.

Abstract The aim of this study was to evaluate in vitro the cytotoxicity on human periodontal ligament fibroblasts of three root-end filling materials: MTA Angelus(r), EndoSequence Root Repair Material Putty(r) and Super EBA(r). A primary culture of human periodontal ligament fibroblasts was previously obtained in order to evaluate the cytotoxicity of the three extracts from the root-end filling materials after 2 and 7 days of setting. Serial dilutions of these extracts (1:1, 1:2, 1:4 and 1:8) were evaluated at 1, 3 and 7 days using the methyl-thiazol-tetrazolium (MTT) colorimetric assay. Cell viability was evaluated as percentage of the negative control group, which represented 100% cell viability. Statistical analyses were done with t-test, ANOVA and Kruskal-Wallis test at a significance level of 5%. It was found that the main difference among root-end filling materials was in the higher dilutions (p<0.05), but there was a similar behavior in lower dilutions (p>0.05). Cell viability of MTA Angelus(r) was superior for 2-day setting (p<0.05), compared with the other two root-end fillings. There were no statistically significant differences between 7-day set MTA Angelus(r) and EndoSequence Root Repair Material Putty(r). Super EBA(r) showed the lowest percentage of cell viability at higher dilutions (p<0.05). Therefore, MTA Angelus(r) and EndoSequence Root Repair Material Putty(r) were less cytotoxic in the highest dilution (1:1) compared with Super EBA(r).


Resumo O objetivo deste trabalho foi avaliar in vitro a citotoxicidade em fibroblastos do ligamento periodontal humano de três cimentos de retrobturação: MTA Angelus(r), EndoSequence Root Repair Material Putty(r) e Super EBA(r). Uma cultura de fibroblastos primários do ligamento periodontal humano foi obtida anteriormente a fim de avaliar a citotoxicidade dos três extratos dos cimentos de retrobturação após 2 e 7 dias de endurecimento. As diluições em série destes extratos (1:1, 1:2, 1:4 e 1:8) foram avaliados em 1, 3 e 7 dias empregando o ensaio colorimétrico metil-tiazol-tetrazólio (MTT). A viabilidade celular foi calculada em base da porcentagem do grupo de controle negativo, que representou 100% de viabilidade de células. As análises estatísticas foram realizadas com o teste t, ANOVA e teste de Kruskal-Wallis a um nível de significância de 5%. Verificou-se que a principal diferença entre os cimentos de retrobturação estava nas diluições mais elevadas (p<0,05) e houve um comportamento semelhante nas diluições mais baixas (p>0,05). A viabilidade celular dos fibroblastos do ligamento periodontal humano foi superior para MTA Angelus(r) de 2 dias de endurecimento (p<0,05), em comparação com os outros materiais de retrobturação. Não houve diferença significante entre MTA Angelus(r) e EndoSequence Root Repair Material Putty(r) de 7 dias de endurecimento. Super EBA(r) mostrou a menor percentagem da viabilidade celular nas diluições mais altas (p<0,05). Portanto, os cimentos de retrobturação MTA Angelus(r) e EndoSequence Root Repair Material Putty(r) foram menos citotóxicos na diluição mais alta (1:1) em comparação com Super EBA(r).

ABSTRACT

Abstract When seeking orthodontic treatment, many adolescents and adult patients present with deciduous teeth. Naturally, deciduous teeth will inevitably undergo exfoliation at the expected time or at a later time. Apoptosis is the biological trigger of root resorption. In adult patients, deciduous teeth should not be preserved, as they promote: infraocclusion, traumatic occlusion, occlusal trauma, diastemata and size as well as morphology discrepancy malocclusion. Orthodontic movement speeds root resorption up, and so do restoring or recontouring deciduous teeth in order to establish esthetics and function. Deciduous teeth cells are dying as a result of apoptosis, and their regeneration potential, which allows them to act as stem cells, is limited. On the contrary, adult teeth cells have a greater proliferative potential. All kinds of stem cell therapies are laboratory investigative non authorized trials.


Resumo Muitos adolescentes e adultos, ao procurar pelo tratamento ortodôntico, apresentam dentes decíduos persistentes. Naturalmente, os dentes decíduos ou se esfoliam na época esperada ou mais tardiamente, de forma inevitável. A apoptose é o gatilho biológico da rizólise. Em adultos, os dentes decíduos não devem ser preservados, pois promovem: infraoclusão, oclusão traumática, trauma oclusal, além de diastemas e má oclusão por discrepância de tamanho e morfologia. O movimento ortodôntico acelera o processo de rizólise, assim como restaurar ou reanatomizar dentes decíduos para inseri-los em uma estética e função. As células dos dentes decíduos estão morrendo por apoptose e seu potencial regenerativo para atuarem como células-tronco tem limitações, ao contrário das células de dentes adultos, que têm maior potencial proliferativo. Todas as terapias com células-tronco ainda são laboratoriais e se enquadram como ensaios investigativos não autorizados.

ABSTRACT Introduction: Osteoclasts and osteoblasts are responsible for regulating bone homeostasis during which the trace element zinc has been shown to exert a cumulative effect on bone mass by stimulating osteoblastic bone formation and inhibiting osteoclastic bone resorption. Objective: The aim of the present study was to investigate the effects of zinc (Zn) on orthodontic tooth movement (OTM) in a rat model. Material and Methods: A total of 44 male Wistar rats were divided into four groups of 11 animals each and received 0, 1.5, 20 and 50 ppm Zn in distilled water for 60 days. In the last 21 days of the study, nickel-titanium closed coil springs were ligated between maxillary right incisors and first molars of all rats, and tooth movement was measured at the end of this period. Histological analysis of hematoxylin/eosin slides was performed to assess root resorption lacunae, osteoclast number and periodontal ligament (PDL) width. Results: Mean OTM was calculated as 51.8, 49.1, 35.5 and 45 µm in the 0, 1.5, 20 and 50 ppm zinc-receiving groups, respectively. There were no significant differences in neither OTM nor histological parameters among the study groups (p > 0.05). Conclusion: According to the results obtained in the current investigation, increase in supplementary zinc up to 50 ppm does not affect the rate of OTM neither bone and root resorption in rats.


RESUMO Introdução: os osteoclastos e os osteoblastos são responsáveis por regular a homeostase óssea, processo no qual o oligoelemento zinco tem demonstrado exercer um efeito cumulativo sobre a massa óssea, estimulando a formação óssea osteoblástica e inibindo a reabsorção óssea osteoclástica. Objetivo: o objetivo do presente estudo foi investigar os efeitos do zinco (Zn) sobre a movimentação dentária ortodôntica (MDO) em ratos. Métodos: um total de 44 ratos Wistar machos foi dividido em quatro grupos de 11 animais cada, os quais receberam 0; 1,5; 20 e 50ppm de zinco diluído em água destilada, durante 60 dias. Nos últimos 21 dias do estudo, molas helicoidais fechadas de níquel-titânio foram instaladas entre os incisivos direitos e os primeiros molares superiores de todos os ratos, e a movimentação dentária foi medida ao final desse período. Foi realizada análise histológica de cortes corados por hematoxilina-eosina, para avaliar as lacunas de reabsorção radicular, o número de osteoclastos e a espessura do ligamento periodontal. Resultados: as médias da MDO foram estimadas em 51,8; 49,1; 35,5 e 45µm no grupos que receberam, respectivamente, 0; 1,5; 20 e 50ppm de zinco. Não houve diferença significativa entre os grupos experimentais, nem quanto à MDO, nem quanto aos parâmetros histológicos (p > 0,05). Conclusão: segundo os resultados obtidos na presente investigação, verificou-se que um aumento na dose de suplementação com zinco para 50ppm não afeta nem o índice de MDO, nem a reabsorção óssea ou radicular em ratos.

ABSTRACT

OBJECTIVE: This study aimed to assess tissue changes during orthodontic movement after binge-pattern ethanol 20% exposure. METHODS: Male Wistar rats (n = 54) were divided into two groups. The control group (CG) received 0.9% saline solution, while the experimental group (EG) received 20% ethanol in 0.9% saline solution (3 g/kg/day). On the 30th day, a force of 25 cN was applied with a nickel-titanium closed coil spring to move the maxillary right first molar mesially. The groups were further divided into three subgroups (2, 14 and 28 days). Tartrate-resistant acid phosphatase and picrosirius were used to assess bone resorption and neoformation, respectively. Data were compared by two-way ANOVA, Tukey's HSD, Games-Howell and chi-square test. Significance level was set at 5%. RESULTS: There was a decrease in the number of osteoclasts in EG at day 28. The percentage of collagen showed no interaction between group and time. CONCLUSION: Binge-pattern 20% ethanol promoted less bone resorption at the end of tooth movement, thereby suggesting delay in tooth movement. .


OBJETIVO: objetivou-se avaliar as alterações teciduais decorrentes da administração de etanol a 20% no padrão binge, durante o movimento ortodôntico. MÉTODOS: foram utilizados ratos Wistar machos (n = 54), divididos em dois grupos, sendo Grupo Controle (GC), com administração de soro fisiológico a 0,9%; e Grupo e Experimental (GE), com administração de etanol a 20% em soro fisiológico a 0,9%, no volume de 3g/kg/dia. Após o 30º dia de administração, foi aplicada força de 25cN com mola fechada de níquel-titânio para mover o primeiro molar superior direito para mesial. Os grupos foram subdivididos nos subgrupos 2, 14 e 28 dias, correspondendo ao número de dias de movimentação dentária. Utilizou-se as colorações de fosfatase ácida-tartarato resistente e picrosírius para avaliar reabsorção óssea e neoformação óssea, respectivamente. Os dados foram comparados por ANOVA a dois critérios, Tukey HSD e Games-Howell, ao nível de significância de 5%. RESULTADOS: verificou-se diminuição no número de osteoclastos no GE II no 28º dia. A percentagem de colágeno não demonstrou alteração na interação grupo x tempo. CONCLUSÕES: o etanol no padrão binge a 20% promoveu menor reabsorção óssea no final da movimentação dentária, sugerindo atraso na movimentação dentária. .

Cemento-ossifying fibroma is a relatively rare, benign, non-odontogenic tumour of the jaws, regarded as a subdivision of fibro-osseous lesions. The usual age of occurrence is between 20 and 40 years. The female:male ratio is 5:1, usual site being the posterior mandible. The aim of this report to present the clinical and radiological features and management of a 32-year-old female Libyan patient who presented with an asymptomatic giant swelling of three years' duration in the left mandible, the diagnosis of which was confirmed by histopathology. Cemento-ossifying fibroma (COF) is a benign, asymptomatic lesion of the jaws characterized by the production of well-demarcated bone of slow growth. It typically affects females aged between 20 and 40 years, in the premolar and molar area, causing a painless swelling, of slow, expansile growth. The periodontal ligament contains both bone and cementum. The pathogenesis of extraosseous COF, where there is no periodontal tissue, as suggested by Cakir and Karadayi, originates in embryonic nests and the ectopic periodontal membrane, as suggested by Brademann et al. in their cytogenetic studies. Depending on the stage of maturation, cemento-ossifying fibroma may range from radiolucent, through mixed, to completely radio-opaque. Histopathologically, it appears as well-circumscribed, occasionally encapsulated with various amounts of bony trabecular/cementum formation in a fibrous stroma... (AU)


Fibroma Cemento-ossificante é um tumor benigno relativamente raro, não odontogênico dos maxilares, considerado como subdivisão de lesões fibro-ósseas. A idade usual de ocorrência é entre 20 e 40 anos. Há predileção do sexo feminino na proporção 5:1, e da região posterior da mandíbula. Esse relato de caso apresenta uma paciente libanês de 32 anos, que se apresentou com aumento grande de volume, achados clínicos e radiológicos assintomáticos na mandíbula esquerda com duração de três anos, o diagnóstico histopatológico confirma o diagnóstico clínico. Fibroma cemento-ossificante (COF) é caracterizado pela produção óssea de crescimento lento, bem demarcada, assintomática, benigna dos maxilares. Geralmente afeta mulheres entre a idade de 20 a 40 anos na área molares e pré-molares, causando um edema, indolor, crescimento lento e expansível. A patogênese das lesões extras ósseas do COF podem não apresentar tecidos periodontais, como sugerido por Cakir e Karadayi e sim células embrionárias e membrana periodontal ectópica como sugerido por Brademann et al. em seus estudos citogenéticos. Dependendo do estágio de maturação, fibroma cemento-ossificante pode ter umavariedaderadiolúcida, mista ou completamente radiopaca. OBJECTIVE: This article aimed at identifying the expression of fibroblast growth factor-2 (FGF-2) and vascular endothelial growth factor (VEGF) in the tension and pressure areas of rat periodontal ligament, in different periods of experimental orthodontic tooth movement. METHODS: An orthodontic force of 0.5 N was applied to the upper right first molar of 18 male Wistar rats for periods of 3 (group I), 7 (group II) and 14 days (group III). The counter-side first molar was used as a control. The animals were euthanized at the aforementioned time periods, and their maxillary bone was removed and fixed. After demineralization, the specimens were histologically processed and embedded in paraffin. FGF-2 and VEGF expressions were studied through immunohistochemistry and morphological analysis. RESULTS: The experimental side showed a higher expression of both FGF-2 and VEGF in all groups, when compared with the control side (P < 0.05). Statistically significant differences were also found between the tension and pressure areas in the experimental side. CONCLUSION: Both FGF-2 and VEGF are expressed in rat periodontal tissue. Additionally, these growth factors are upregulated when orthodontic forces are applied, thereby suggesting that they play an important role in changes that occur in periodontal tissue during orthodontic movement. .


OBJETIVO: o objetivo desse estudo foi identificar a expressão do fator de crescimento de fibroblastos 2 (FGF-2) e do fator de crescimento vascular endotelial (VEGF) nos lados de tensão e pressão do ligamento periodontal de ratos, durante movimento ortodôntico experimental, em diferentes períodos de tempo. MÉTODOS: uma força ortodôntica de 0,5N foi aplicada no primeiro molar superior direito de 18 ratos Wistar machos, por períodos de 3 (grupo I), 7 (grupo II) e 14 dias (grupo III). O primeiro molar do lado oposto foi utilizado como controle. Os animais foram sacrificados nos períodos de tempo mencionados, sendo a arcada superior removida e fixada. Após a desmineralização, os espécimes foram processados histologicamente e embebidos em parafina. A expressão do FGF-2 e do VEGF foram estudadas por meio de análise imuno-histoquímica. RESULTADOS: o ligamento periodontal dos dentes submetidos à movimentação ortodôntica mostraram maior expressão tanto de FGF-2 quanto de VEGF, em todos os grupos experimentais, quando comparados com os dentes do lado controle (p < 0,05). Diferenças estatisticamente significativas entre os lados de tensão e pressão também foram encontradas nos dentes submetidos à movimentação ortodôntica. CONCLUSÕES: tanto o FGF-2 quanto o VEGF são expressos no tecido periodontal de ratos, e esses fatores de crescimento são aumentados quando forças ortodônticas são aplicadas, sugerindo que esses desempenham um papel importante na reorganização do periodonto durante o movimento ortodôntico. .

ABSTRACT

A common question about root resorption is raised in orthodontic practice: What is more important, the intensity of force or its distribution along the root, periodontal and alveolar structures? Diffuse distribution of forces applied to periodontal tissues during tooth movement tends not to promote neither extensive areas of cell matrix hyalinization nor significant death of cementoblasts that lead to root resorption. However, focal distribution or concentration of forces within a restricted area - as it occurs in tipping movements, even with forces of lower intensity - tend to induce extensive areas of hyalinization and focal death of cementoblasts, which is commonly associated with root resorption. In tipping movements, the apical regions tend to concentrate more forces in addition to wounding the cementoblasts due to the smaller dimension of their root structure as well as their cone shape. For this reason, there is an increase in root resorption. In the cervical region, on the other hand, the large area resulting from a large diameter and bone crown deflection tends to reduce the effects of forces, even when they are more concentrated, thus rarely inducing death of cementoblasts and root resorption.


Um questionamento comum sobre as reabsorções radiculares na prática ortodôntica: "O que é mais importante? A intensidade das forças aplicadas ou sua distribuição ao longo das estruturas radiculares, periodontais e alveolares?" A distribuição difusa das forças aplicadas sobre os tecidos periodontais durante o movimento dentário de corpo tende a não promover extensas áreas de hialinização da matriz extracelular, nem morte significativa de cementoblastos que levariam à reabsorção radicular. Porém, a distribuição focal ou concentração de forças - como nas inclinações, mesmo nas de menor intensidade - em uma área restrita tende a induzir áreas extensas de hialinização e morte focal de cementoblastos, associando-se mais comumente à reabsorção radicular. Nos movimentos de inclinação, as áreas apicais, por sua menor dimensão da estrutura radicular e sua forma cônica, tendem a concentrar mais ainda as forças e lesar cementoblastos, aumentando a frequência das reabsorções radiculares. Na região cervical, a maior área decorrente do maior diâmetro e a deflexão óssea da crista óssea tendem a reduzir os efeitos das forças, mesmo quando mais concentradas, muito raramente induzindo a morte de cementoblastos e reabsorções radiculares.

ABSTRACT

OBJECTIVE: The objective of this study was to assess the histological alterations that occurred in the periodontal area of rat molars submitted to induced tooth movement (ITM) right after an intentional trauma (subluxation). METHODS: Forty adult male Wistar rats (Rattus norvegicus albinus) were selected. The animals were divided into eight groups (n = 5), according to the combination of variables: Group 1 - control (neither trauma nor ITM); Group 2 - ITM; Groups 3, 4, 5 and 6 - dentoalveolar trauma groups corresponding, respectively, to 1, 3, 8 and 10 days after trauma; Groups 7 and 8 - the animals' molars were subjected to a 900 cN impact and, one and three days after the trauma event, tooth movement was induced. The rats' maxillary first molars were mesially moved during seven days with a closed coil (50 cN). After the experimental period of each group, the animals were sacrificed by anesthetic overdose and the right maxillas were removed and processed for histological analysis under light microscopy. RESULTS: In the animals of group 3, 4, 5 and 6, the histological alterations were not very significant. Consequently, the effect of induced tooth movement right after a subluxation event (groups 7 and 8) was very similar to those described for Group 2. CONCLUSION: There was no difference in the quality of periodontal repair when ITM was applied to teeth that had suffered a subluxation trauma. .


OBJETIVO: avaliar as alterações histológicas ocorridas na área periodontal de molares de ratos submetidos à movimentação dentária induzida (MDI), logo após um trauma intencional (subluxação). MÉTODOS: quarenta ratos Wistar machos adultos (Rattus norvegicus albinus) foram selecionados. Os animais foram divididos em oito grupos (n = 5), de acordo com a combinação das variáveis: Grupo 1 - controle (sem trauma e sem MDI); Grupo 2 - MDI; Grupos 3, 4, 5 e 6 - grupos de trauma dentoalveolar correspondendo, respectivamente, para 1, 3, 8 e 10 dias após o trauma; Grupos 7 e 8 - os molares murinos foram submetidos a um impacto de 900cN e, de um e três dias após o evento trauma, o movimento do dente foi induzido. Os primeiros molares superiores dos animais foram movidos mesialmente durante sete dias, com uma mola fechada (50cN). Após período experimental de cada grupo, os animais foram sacrificados por overdose anestésica e as maxilas direitas foram removidas e processadas para análise histológica qualitativa. RESULTADOS: nos animais dos grupos 3, 4, 5 e 6, as alterações histológicas não foram muito significativas. Consequentemente, o efeito do movimento dentário induzido logo após um evento de subluxação (grupos 7 e 8) foi muito semelhante ao descrito para o grupo 2. CONCLUSÃO: não houve diferença na qualidade do reparo periodontal quando a MDI foi aplicada aos dentes que sofreram um trauma de subluxação. .

ABSTRACT

The aim was to correlate alveolar bone loss in the buccal/palatal and the mesial/distal surfaces of upper molars in rats. Thirty-three, 60-day-old, male Wistar rats were divided in two groups, one treated with alcohol and the other not treated with alcohol. All rats received silk ligatures on the right upper second molars for 4 weeks. The rats were then euthanized and their maxillae were split and defleshed with sodium hypochlorite (9%). The cemento-enamel junction (CEJ) was stained with 1% methylene blue and the alveolar bone loss in the buccal/palatal surfaces was measured linearly in 5 points on standardized digital photographs. Measurement of the proximal sites was performed by sectioning the hemimaxillae, restaining the CEJ and measuring the alveolar bone loss linearly in 3 points. A calibrated and blinded examiner performed all the measurements. Intraclass Correlation Coefficient revealed values of 0.96 and 0.89 for buccal/lingual and proximal surfaces, respectively. The Pearson Correlation Coefficient (r) between measurements in buccal/palatal and proximal surfaces was 0.35 and 0.05 for the group treated with alcohol, with and without ligatures, respectively. The best correlations between buccal/palatal and proximal surfaces were observed in animals not treated with alcohol, in sites both with and without ligatures (r = 0.59 and 0.65, respectively). A positive correlation was found between alveolar bone loss in buccal/palatal and proximal surfaces. The correlation is stronger in animals that were not treated with alcohol, in sites without ligatures. Areas with and without ligature-induced periodontal destruction allow detection of alveolar bone loss in buccal/palatal and proximal surfaces.

ABSTRACT

The interrelationship between periodontal and endodontic disease has aroused confusion, queries and controversy. Differentiating between periodontal and endodontic problems can be difficult. A symptomatic tooth may have pain of periodontal and/or pulpal origin. The nature of that pain is often the first clue in determining the etiology of such a problem. Radiographic and clinical evaluation can help clarify the nature of the problem. In some cases, the influence of pulpal pathology may create periodontal involvement. In others, periodontal pathology may create pulpal pathology. This review article discusses the various clinical aspects to be considered for accurately diagnosing and treating endo-perio lesions.

OBJECTIVES: The aim of this study was to test the efficacy of a locally applied 8.5 percent nanostructured doxycycline (DOX) gel in preventing alveolar bone loss in experimental periodontal disease (EPD) in rats by using the tapping mode atomic force microscopy (AFM). MATERIAL AND METHODS: EPD was induced in 24 Wistar rats. Animals were treated with the doxycycline gel topically, immediately after EPD induction, and 3 times a day during 11 days. Four groups (n=6) were formed as follows: Naïve group (animals not subjected to EPD nor treated); non-treated (NT) group (animals subjected to EPD, but not treated); vehicle gel (VG) group (animals subjected to EPD and treated with topical gel vehicle); and DOX group (test group): animals subjected to EPD and treated with the 8.5 percent DOX gel. In order to investigate topographical changes in histological sections, a novel simple method was used for sample preparation, by etching sections from paraffin-embedded specimens with xylol. RESULTS: Comparing the AFM images, several grooves were observed on the surface of the alveolar bone and other periodontal structures in the NT and VG groups, with significantly greater depths when compared to the DOX group (p<0.05). CONCLUSIONS: Periodontal structures were brought into high relief confirming to be a simple and cost-effective method for AFM imaging with ultrastructural resolution. The doxycycline gel was able to afford periodontal surface preservation, with flatter grooves.

ABSTRACT

This study aimed to assess the response of apical and periapical tissues of dogs' teeth after root canal filling with different materials. Forty roots from dogs' premolars were prepared biomechanically and assigned to 4 groups filled with: Group I: commercial calcium hydroxide and polyethylene glycol-based paste (Calen®) thickened with zinc oxide; Group II: paste composed of iodoform, Rifocort® and camphorated paramonochlorophenol; Group III: zinc oxide-eugenol cement; Group IV: sterile saline. After 30 days, the samples were subjected to histological processing. The histopathological findings revealed that in Groups I and IV the apical and periapical regions exhibited normal appearance, with large number of fibers and cells and no resorption of mineralized tissues. In Group II, mild inflammatory infiltrate and mild edema were observed, with discrete fibrogenesis and bone resorption. Group III showed altered periapical region and thickened periodontal ligament with presence of inflammatory cells and edema. It may be concluded that the Calen paste thickened with zinc oxide yielded the best tissue response, being the most indicated material for root canal filling of primary teeth with pulp vitality.


O objetivo deste estudo foi avaliar a resposta dos tecidos apicais e periapicais de dentes de cães, após obturação dos canais radiculares com diferentes materiais indicados para dentes decíduos. Foram utilizados pré-molares de cães, totalizando 40 raízes que, após pulpectomia e preparo biomecânico, foram divididas em 4 grupos, nos quais os canais radiculares foram obturados com os seguintes materiais: Grupo I - pasta comercial composta de hidróxido de cálcio e polietileno glicol 400 (Calen®) espessada com óxido de zinco; Grupo II - pasta composta de iodofórmio, Rifocort® e paramonoclorofenol canforado; Grupo III - cimento de óxido de zinco e eugenol; e Grupo IV - solução salina. Decorridos 30 dias, as peças foram submetidas ao processamento histológico. De acordo com os resultados da análise histopatológica observou-se que nos Grupos I e IV as regiões apical e periapical apresentaram aspecto de normalidade, com grande número de fibras e células e ausência de reabsorção dos tecidos mineralizados. No Grupo II observou-se infiltrado inflamatório e edema leves, com discreta fibrogênese e reabsorção óssea. O Grupo III apresentou alteração na região periapical e ligamento periodontal ampliado, com presença de células inflamatórias e edema. Os resultados obtidos permitiram concluir que a pasta Calen espessada com óxido de zinco apresentou a melhor resposta tecidual, sendo a mais indicada para a obturação de canais radiculares de dentes decíduos com vitalidade pulpar.

ABSTRACT

This study evaluated histopathologically different methods of experimental induction of periapical periodontitis. The radiographic and microbiological evaluations have been performed in a previous investigation. Fifty-seven root canals from dogs' teeth were assigned to 4 groups. In GI (n=14) and GII (n=14), the root canals were exposed to oral environment for 180 days; in GIII (n=14) and GIV (n=15) the root canals were exposed for 7 days and then the access cavities were restored and remained sealed for 53 days. The root apices of GI and GIII were perforated, whilst those of GII and GIV remained intact. After induction of periapical periodontitis, the dogs were euthanized. Serial sections were obtained and stained with hematoxylin and eosin. Data of the histopathological evaluation were submitted to Kruskal-Wallis and Dunn's tests at 5 percent significance level. The inflammatory periapical reaction and resorption of mineralized tissues were less intense in GII than in the other groups (p<0.05). There was no histopathological difference among the experimentally induced periapical lesions in the teeth with coronal sealing. On the other hand, when coronal sealing was not performed, greater intensity of induced periapical periodontitis was observed in the teeth with apical perforation.


O objetivo deste estudo foi a avaliação de diferentes métodos de indução de lesões periapicais. Cinqüenta e seis canais radiculares de dentes de cães foram divididos em 4 grupos. No GI (n=14) e no GII (n=14), os canais radiculares foram expostos à cavidade bucal por 180 dias; no GIII (n=14) e no GIV (n=15) os canais radiculares foram expostos por 7 dias e então as aberturas coronárias foram restauradas e permaneceram seladas por 53 dias. Os ápices radiculares do GI e GIII foram perfurados, enquanto os do GII e do GIV foram mantidos intactos. Após indução das lesões periapicais, os cães foram mortos. Cortes seriados foram obtidos e corados por hematoxilina e eosina. Os dados da análise histopatológica foram submetidos ao teste de Kruskal-Wallis e Dunn com nível de significância de 5 por cento. A reação inflamatória periapical e reabsorção dos tecidos mineralizados foram menos intensos no GII que nos demais grupos (p<0,05). Não houve diferença histopatológica entre as lesões periapicais induzidas nos dentes com selamento coronário. Por outro lado, quando o selamento coronário não foi realizado, maior intensidade de lesão periapical induzida foi observada nos dentes com perfuração apical.

ABSTRACT

The aim of this article is to show the clinical and histochemical alterations of the first periodontal ligament, on the right side, after upper molars teeth extraction on the left side in gerbils. After two months, the periodontal ligaments were removed and processed for histochemical analysis. The data showed that TRAP reaction was able to evidence the osteoclastic activity in the hyperfunction hemimandible, right side, explaining the functional changes in the periodontal ligament after teeth extraction, and a little gingival recession and radicular exposure of teeth without function was observed at inferior molars of the left side.


El objetivo de este artículo es mostrar las alteraciones clínicas e histoquímicas del primer ligamento periodontal del lado derecho, después de la extracción del molar superior izquierdo en gerbiles {Meriones unguiculatus). Luego de dos meses, los ligamentos periodontales fueron retirados y procesados para el análisis histoquímico. Los resultados mostraron que la reacción de TRAP es capaz de evidenciar la actividad osteoclástica en la hiperfunción de la semimandíbula derecha, explicando los cambios funcionales del ligamento periodontal después de la extracción dental, siendo observada una pequeña recesión gingival y exposición radicular de los dientes sin función, en los molares inferiores izquierdos.

The purpose of this study was to evaluate the influence of coronal leakage on the healing of dogs' periapical tissues after root canal filling, post space preparation and protection or not with a temporary sealer plug. Forty root canals of dogs' teeth were instrumented and filled by the lateral condensation technique with gutta-percha points and Endomethasone or CRCS sealers. After post space preparation, the remaining filling material was protected or not with a plug of temporary Coltosol sealer and exposed to the oral environment for 90 days. Thereafter, the animals were sacrificed and the specimens were removed and prepared for histomorphological and histobacteriological analysis. The findings revealed 35 percent of microbial leakage in the groups without plugs and 15 percent of leakage in the groups with plugs. Statistical analysis showed that the use of a Coltosol plug improved significantly the histomorphological results regardless of the type of root canal sealer (p=0.05) and that CRCS and Endomethasone sealers showed similar results (p>0.05).


O propósito deste trabalho foi avaliar a influência da infiltração coronária no reparo dos tecidos periapicais após obturação dos canais radiculares, preparo para pino e proteção ou não de um "plug" de cimento temporário. Quarenta canais de dentes de cães foram instrumentados e obturados pela técnica da condensação lateral ativa com cones de guta-percha e os cimentos Endomethasone e CRCS. Após preparo para pino os remanescentes do material obturador foram protegidos ou não com um "plug" do cimento temporário Coltosol e expostos ao meio oral por 90 dias. Decorrido este período, os animais foram mortos e os espécimes foram removidos e preparados para análises histomorfológica e histobacteriológica. Foi observado 35 por cento de casos de infiltração bacteriana nos grupos sem "plug" e 15 por cento nos grupos com "plug". Concluiu-se estatisticamente que o "plug" de Coltosol foi eficiente no controle da infiltração coronária de microorganismos (p=0,05), e que os cimentos CRCS e Endomethasone apresentaram resultados semelhantes (p>0,05).

The rat model is widely used in periodontal research and the quality of histological sections is essential. The purpose of this study was to evaluate the histological characteristics of periodontal tissues in Wistar rat maxillae, with different times of fixation and decalcified by nitric acid or formic acid (Anna Morse Solution). Fifteen rats were used. Fixation was performed for 24, 48 and 72 hours. The maxillae were hemi-sectioned and each part was decalcified either in nitric acid for 7 days or in Anna Morse solution for 35 days. Two trained and blinded examiners performed the evaluation. Fourty eight hours of fixation and decalcification with Anna Morse solution showed more clear characteristics of the epithelium-connective tissue interface and of the periodontal structures. Mean measurements between the cementum-enamel junction and the bone crest varied in the different experimental times from 176.5 (± 60.45) to 210.94 (± 39.33) pixels on the buccal aspect, and from 199.69 (± 38.33) to 298.55 (± 70.81) pixels on the palatal aspect, with no statistically significant differences (ANOVA, p > 0.05). In the same fixation period, decalcification with nitric acid or Anna Morse solution did not display any statistically significant differences. It may be concluded that for a qualitative histological analysis, fixation should preferably be for 48 hours and the demineralization should be made by Anna Morse solution. For a histomorphometric analysis, the decalcification solution does not interfere in the results.


O modelo rato é extensamente usado na pesquisa periodontal, e a qualidade dos cortes histológicos é essencial. A proposta deste estudo foi avaliar as características histológicas dos tecidos periodontais nas maxilas de ratos Wistar, após diferentes períodos de fixação e descalcificação pelo ácido nítrico ou pelo ácido fórmico (Solução de Anna Morse). Quinze ratos foram usados. A fixação foi realizada nos períodos de 24, 48 e 72 horas. As maxilas foram divididas e parte foi descalcificada em ácido nítrico durante 7 dias e parte com solução de Anna Morse por 35 dias. Dois examinadores treinados e cegos executaram a avaliação. Quarenta e oito horas de fixação e descalcificação com solução de Anna Morse mostraram características mais evidentes da interface epitélio-conjuntivo, assim como das estruturas periodontais. As médias, por vestibular, entre a junção cemento-esmalte e a crista óssea nos diferentes tempos experimentais variaram entre 176,5 (± 60,45) e 210,94 (± 39,33) "pixels", e, na face palatina, entre 199,69 (± 38,33) e 298,55 (± 70,81) "pixels", sem nenhuma diferença estatisticamente significativa (ANOVA, p > 0,05). No mesmo período de fixação, a descalcificação com ácido nítrico ou solução de Anna Morse não mostrou diferenças estatisticamente significantes. Pode-se concluir que, para a análise histológica qualitativa, a fixação deve ser preferivelmente em 48 horas e a desmineralização por solução de Anna Morse. Para a análise histo-morfométrica, a solução descalcificadora não interferiu nos resultados.

ABSTRACT

El mantenimiento del ligamento periodontal sin vitalidad en la superficie radicular puede influenciar en el proceso de reparación en el reimplante dental, por lo tanto puede ser sustituido por tejido óseo o comenzar un proceso de reabsorción inflamatória. Por lo tanto, muchas formas de retiro de este ligamento se han estudiado. Es propuesta de ese trabajo evaluar, por medio de cortes histológicos, los resultados del tratamiento de la superficie de los dientes sometidos a exodoncias y mantenerlo en medio ambiente por 6 horas. Para la realización del experimiento, se utilizaron 15 ratones, divididos en tres grupos, con 5 dientes cada uno: Grupo I, suero fisiológico por tres minutos; Grupo II, ácido cítrico (pH 1.0) por tres minutos; Grupo III, fricción de la superficie radicular con gaza esteril humedecida con ácido cítrico (pH1.0) por um minuto. Después del procesamiento laboratorial de rutina los cortes fueron sometidos a análisis cuantitativo y cualitativo (Software ImageLab - Diracom 3). En todos los grupos fueron observados restos del ligamento periodontal en toda la extensión palatina de la superfície radicular. El grupo III tuvo la mayor área de tejido con diferencia estadisticamente significante (p> 0.001). Fue posible concluir que el ácido cítrico no fue capaz de remover el ligamento periodontal necrosado de dientes de ratón después de seis horas de resequedad, en ninguna de las formas de aplicación utilizada.


The maintenance of the periodontal ligament without vitality on the root surface can influence in the process of repair in the dental replantation, therefore it can be substituted by bone tissue or to give beginning to a process of inflammatory resorption. Therefore, many forms of removal of this ligament have been studied. It is intention of this work to evaluate, by means of histological study, the results of the treatment of the tooth surface in dental extraction and left in environment for 6 hours. For the accomplishment of the experiment 15 mouse, divided in three groups will be used, with 5 teeth in each: Group I, salt solution per three minutes; Group II, acid citric (pH 1,0) per three minutes e; Group III, friction of the root surface with acid humidified barren gauze with citric (pH 1,0) per one minute. After the laboratorial processing of routine the cuts had been submitted to the qualitative and quantitative analysis (Software ImageLab - Diracom 3). In all the groups had been observed remaining of the periodontal ligament in all palatal extension of the root s surface. Group III showed to greater tissue area with statistic significant difference (p > 0.001). It was possible to conclude that acid the citric one was not capable to after remove the rat periodontal tooth ligament necrosis six hours of drying, in none of the forms of used application.


A manutenção do ligamento periodontal sem vitalidade sobre a superfície radicular pode influenciar no processo de reparo no reimplante dentário, pois pode ser substituído por tecido ósseo ou dar início a um processo de reabsorção inflamatória. Por isso, muitas formas de remoção desse ligamento têm sido estudadas. É propósito de este trabalho avaliar, por meio de cortes histológicos, os resultados do tratamento da superfície de dentes avulsionados cirurgicamente e deixados em meio ambiente por 6 horas com soro fisiológico e ácido cítrico (pH 1.0). Para a realização do experimento serão empregados 15 ratos, divididos em três grupos, com 5 dentes em cada: Grupo I, soro fisiológico por três minutos; Grupo II, ácido cítrico (pH 1.0) por três minutos e; Grupo III, fricção da superfície radicular com gaze estéril umedecida com ácido cítrico (pH 1.0) por um minuto. Após o processamento laboratorial de rotina os cortes foram submetidos à análise qualitativa e quantitativa (Software ImageLab - Diracom 3). Em todos os grupos foram observados remanescentes do ligamento periodontal em toda extensão palatina da superfície radicular. O grupo III mostrou maior área de tecido com diferença estatisticamente significante (p> 0.001). Foi possível concluir que o ácido cítrico não foi capaz de remover o ligamento periodontal necrosado de dentes de rato após seis horas de ressecamento, em nenhuma das formas de aplicação utilizada.

ABSTRACT

A avaliaçäo do desempenho das Redes Neurais (RN) na identificaçäo de sítios periodontais sem (SPI) e com perda de inserçäo (CPI) baseou-se nas observaçöes clínicas de uma amostra de 2701 sítios de 19 indivíduos monitorados por um período de 18 meses. Os dados referentes às medidas sequenciais do nível de inserçäo relativo foram analisados pelos métodos estatísticos de tolerância e regressäo para a determinaçäo de sítios SPI e CPI. Os seis parâmetros clínicos (sítio, dente, nível de inserçäo inicial, idade, sexo e fumo) considerados variáveis input, e os resultados estatísticos, representando a variável output, foram aplicados à RN nos Modelo Tolerância (MT) e Modelo Regressäo (MR). As variáveis input e output de 2188 sítios foram utilizadas na fase de treinamento da RN. Em seguida, somente as variáveis input dos 513 sítios restantes foram apresentadas à rede na fase teste, para verificar o seu treinamento. Os resultados evidenciaram a possibilidade de aplicaçäo das RN em Periodontia para identificar sítios SPI (alta especificidade), a partir de uma amostra representativa
